How do I know if I qualify for Personalized Mentorship?

To qualify for our PERSONALIZED mentorship program, you must be at least 18 years old or older, live in Ohio, and have CareSource, Buckeye, or another type of Medicaid health insurance.

What kind of support does LifeBridge offer?

Finding a new or better job / career
Enrolling in college or career / technical training
Building or fixing your credit
Steps to homeownership
Accessing community resources
Starting or running your own business
Better budgeting and understanding of basic investing
Health, wellness & effective stress management
Relationships skills and guidance
Anxiety and depression management / coping skills

How much time can I get with my Mentor?

If you qualify, you can receive the amount of time per week that will ensure your growth and success. This could range from as little as 30 minutes to upwards of several hours of personal mentoring.

How do I meet with my Mentor once I am assigned one?

You’ll meet with your mentor via phone calls, or computer/video chat. This allows us to serve clients throughout the State of Ohio. Additionally, this enables us to serve you in a way that is convenient and fits into your personal schedule.

What if I do not have Medicaid?

If you have another type of insurance (i.e. private insurance), we do have other options to fit your needs. Simply fill out the Sign Up Form and indicate OTHER. Once you sign up for your assessment, your service options will be discussed.
